What you will need to include on the form

  • Nationality
  • Your National Insurance Number
  • Bank account details
  • Whether you have a driving licence (and details of any convictions)
  • Whether you have transport or not
  • Whether you have attached a CV of not
  • You will need to list your last two employers
  • Whether you have any PPE of your own
  • Minimum hourly rate required
  • Whether you hold a CSCS card
  • Your qualifications – both trade and academic
  • You will need to acknowledge the drugs and alcohol statement
  • You will need to confirm that you are in good health and have no criminal record.
